I think Al Pacino came out of the toilet with a gun shot Camilla once between the eyes and riddled Tony full of bullets then dropped the gun and moved to Italy for awhile.
Actually I like the idea that Tony had a panic attack because that's basically were the whole thing started. The fact is we just don't know being shot is the obvious conclusion but by whom and why.
